The 2021 NYC School Survey is open from May 10 thru June 11. The survey helps school leaders understand what key members of their school community think about the learning environments at their school. Please complete the survey to provide feedback and help us plan for the 2021-2022 academic year.

Parent survey access code is their child’s Students Identification (OSIS) Number. To enter access code, enter “f” followed by the child’s nine-digit OSIS number. Note that “f” must be lowercase.

Enclosed is your copy of the paper 2021 NYC School Survey. For students in blended learning, please drop off their parents completed survey in the provided envelope in Room 227 with Ms. Chu.
