Working Papers — How Do I Get Them?

Working Papers are required for SYEP and other internship programs for students who are least 16 years old. Younger students will also need them if they plan to do paid work afterschool or in the summer. See below for the application form and required documents.

Required Documents to Apply for Working Papers:

1) The application form, with ONLY Part 1 filled out. You can pick up a copy of the application form from your guidance counselor or Ms. Wilson, or download it at: http://www.p12.nysed.gov/sss/documents/AT17-FillableMay21.pdf

2) Your birth certificate, passport, or other federal/state ID (or a clear copy)

3) Your completed Certificate of Physical Fitness (Doctor’s Note). Most pediatricians’ offices have a standard form they will complete; if not, you can use the state’s template at: http://www.p12.nysed.gov/sss/documents/AT-16fillable.pd

***Please note: The Certificate of Physical Fitness is not the same as a physical — it must contain language along the lines of “fit to work”, “healthy for employment”, “can do age-appropriate work”, or something like that.

Once you have your documents ready, come by Ms. Wilson’s office (Room 139) during 4th or 8th period on Tuesday to process your application.
